What"s NEW

■ For those who apply the digital vaccination certificate.
From December 20, 2021, the application which can issue the digital vaccination certificate with 2D barcode is available on your smartphone. My Number Card is required to obtain the digital certificate. For details, please check the following website.

Please also be advised that paper certificate is issued with 2D barcode at each municipality from December 20, 2021.*2

*1 Applications for a digital certificate through the app can only be submitted to the municipality that has started the electronic issuance service. If you apply for a vaccination certificate for international travel using a travel document other than a passport, such as a re-entry permit, or if you are not eligible for the service, you need to apply for a paper certificate at each municipal office (including application by mail).

*2 For a paper certificate, it is issued with 2D barcodes from December 20, even if the application is made before December 20.

■ For those who are planning to use the vaccination certificate around the end of the year and the beginning of the year.
During the year-end and New Year holidays (from December 29, 2021 through January 3, 2022), the vaccination certificate (electronic version) can be issued through the app on your smartphone. The certificate will be issued on the same day if application is accepted without any issues.

On the other hand, if My Number Card is locked after 4-digit PIN is mistaken for a certain number of times in a row, or if the traveler needs to use a travel document other than a passport, the traveler needs to undergo procedures at your municipality.

For this reason, those who plan to use the vaccination certificate for traveling overseas at the end of the year and the beginning of the year, are strongly recommended to apply for the vaccination certificate at your earliest convenience.

In order for people who have already received the COVID-19 vaccine under the Japanese Vaccination Law to be able to use the certificate in various situations, such as when they enter a country during international travel, when they receive preferential treatment to ease the quarantine period upon entry or return to Japan, or when they seek for relaxation of action restrictions in Japan, the vaccination certificate is issued to officially prove that they have received the COVID-19 vaccine.
*From December 20, 2021, application for the vaccination certificate is accepted not only for international travel but also for use in Japan domestic sites.

For domestic use in Japan, “Certificate of vaccination for COVID-19” or “Record of Vaccination for COVID-19” may continue to be used as a valid proof of vaccination.

Overview

Vaccination certificate of COVID-19 is issued as an official proof of COVID-19 vaccination upon receipt of applications from those who are vaccinated at each municipality (including the special wards of Tokyo) under the Japanese Vaccination Law.

Two types of vaccination certificates, one with passport information for international travel & domestic use, and another without passport information for Japan domestic use, can be issued in paper and digital form.

Even after the issuance of new type of certificates, the paper certificate issued in older format is still valid. However, vaccination certificates with 2D barcode may be required in some countries, so please check the latest information at website of countries where you travel or the Ministry of Foreign Affairs when you go abroad.

Please note

  • Taking COVID-19 vaccine is left to the discretion of individuals. Issuance or use of the vaccination certificates itself does not influence each individual’s choice to be vaccinated.
  • Not possessing the vaccination certificate does not restrict international travel.
  • Possessing the vaccination certificate does not guarantee quarantine-free travels nor relaxed quarantine restrictions around the world. The latest information on countries and regions which allow the use of the vaccination certificate is updated at the website of the Ministry of Foreign Affairs of Japan (only in Japanese).
  • Since October 1, 2021, the vaccination certificate as well as vaccination seal and record issued by municipals and clinics have been accepted to receive preferential treatment of new quarantine measures for vaccinated person upon arrival in Japan. As quarantine measures upon arrival in Japan are subject to changes from time to time, please check the latest information at the webpage of サイト内リンク "Border Control Measures".

Application and Issuance

Who's eligible

The Vaccination Certificate of COVID-19 is issued to persons who are vaccinated against COVID-19 under the Japanese Vaccination Law.
※e.g. Vaccinations for priority group such as medical personnel; vaccinations at workplaces; regular vaccinations (vaccinations using vaccination voucher issued by municipalities).

Those who received vaccinations outside Japan* (not vaccinated under the Japanese Vaccination Law) are not eligible.

*  If you temporarily returned to Japan and received vaccination under the program administered by the Ministry of Foreign Affairs of Japan, please contact the Ministry of Foreign Affairs.

Where to apply

Please apply to the municipality where your certificate of residence is registered (usually city, town or village which issued the vaccination vouchers) at the timing of vaccination. If you used the vaccination vouchers of another municipality at the time of vaccination due to moving to another place, please apply to each municipality where your certificate of residence is registered at the timing of vaccination.

Necessary documents to apply for paper certiicate

Please apply to each municipality (including application by mail & web).

For international travel & domestic use in Japan

(1) Application form *1
(2) Valid passport for your travel *2
(3) Document that indicates the vaccination coupon number (such as a part of the “vaccination voucher”, “Certificate of vaccination for COVID-19” with voucher number)*3

For domestic use in Japan

(1) Application form *1
(2) Identification document (name and date of birth is necessary at least)
(3) Document that indicate the vaccination coupon number (such as a part of the “vaccination voucher”, “Certificate of vaccination for COVID-19” with voucher number)*3

 

※Note: Other documents may be required in some cases. For details, please check the website of your municipality.
※Note: For the above document (2) and (3), copies are acceptable.

 

*1  The application form is available at each municipality.

*2  The passport number on the vaccination certificate and the number on the passport which you use for your travel must be the same. If your passport number is changed after the vaccination certificate is issued, please apply for a new certificate with a new passport number. If your passport is in the process of application, please apply for the vaccination certificate after a new passport is issued. Foreign nationals may use passports and similar travel documents issued by foreign governments for the application of the certificate.

*3  If you do not have document (3), you need to present a document that includes your own “My Number” issued by the Government of Japan such as a copy of your certificate of residence with your My Number. If you are unable to present a document which includes your own My Number, you may use your personal identification document which includes your address where you resided at the timing of vaccination.
“Vaccination voucher”, “Certificate of vaccination for COVID-19” with voucher number refers to the following document.

Information included in the Vaccination Certificate

For international travel & domestic use in Japan

The following information is written in both Japanese and English. Anti-counterfeiting measures are taken with digital signature contained in 2D barcode.

(1) Vaccinated person's information (name [Japanese, English], date of birth, etc.)
(2) COVID-19 vaccination record (type of vaccine*1, date of vaccination, etc.)
(3) Passport number and Nationality
(4) 2D barcode which contains the above information and digital signature *2

For domestic use in Japan

The following information is written in both Japanese and English (Name information only in Japanese). Anti-counterfeiting measures are taken with digital signature contained in 2D barcode.

(1) Vaccinated person's information (name [Japanese], date of birth, etc.)
(2) COVID-19 vaccination record (type of vaccine*1, date of vaccination, etc.)

*1  On December 16th, 2021, COVID-19 Vaccine Moderna was approved for additional dose, and its name was changed to “Spikevax” at the same time. Please note that vaccine certificates issued before this change can be used as a proof of vaccination, and that there is no need to reapply for the certificate for this reason. *2  Two types of 2D barcode, ICAO VDS-NC(*4) and SMART Health Cards(*5) are printed.
