本論では,1)自然災害時の精神保健の疫学を概説し,2)近年の大規模自然災害時における精神保健活動の現状を記述したうえで,3)関係者間の役割分担について論じる.最後に,これらすべての支援者にかかわる精神保健上の問題として,4)支援者のストレスについて触れ,各職員および組織レベルでの対応について述べる.
まず,災害後の精神的反応を整理すると,1)適応的反応および回復,2)異常な事態に対する正常な反応,3)精神疾患などに分類される.災害に関連した疾患としては,外傷後ストレス障害(PTSD)が注目されているが,同時にサブクリニカルレベルの気分障害や不安障害,そのほか自殺などの幅ひろいメンタルヘルスの問題に対応する必要がある.このことを踏まえて,通常の保健活動でもあるように,ハイリスク,およびポピュレーション・アプローチというように,それぞれ異なった戦略をとることが,有効な公衆衛生活動になることは平常時と同じである.
災害時の精神保健支援においては,精神保健専門家による臨床的対応および保健的対応においても課題が指摘されている.これらの関係者の円滑な連携のためには,多層的な支援体制をとり,効率的な精神保健資源を配分することが求められる.安否確認,保健・衛生面での安全・安心の確保のために,いち早く地域で住民に対応し,信頼関係を得ている保健師の精神保健上の役割は大きい.住民とのコンタクトの際に,これらの精神保健面での危機状態にある人に接した場合には,精神保健専門家につなげるべきか評価をしつつ,応急的な対応が望まれる.限られた地域の精神保健資源を有効に利用するために,県レベルでの精神保健活動の計画,調整が必要であり,1)精神医療体制の確保,および派遣チーム受け入れの判断,2)精神障害をもつ人で要援護者の把握,ケースへの対応などのハイリスク・アプローチ,3)こころのケアに関する啓発活動といったポピュレーション・アプローチ,4)マスコミ対応などがあげられる.これらの活動を促進するためのガイドラインの更新,そして研修などの技術支援に国や専門機関の積極的関与が求められる.災害時には支援者も大きなストレスを感じるが,個人的のみならず組織的な対応が必要である.災害は不幸な出来事であるが,地域精神保健活動の質の向上の機会にもなり得る.

Abstract
In this article, author reviewed 1)mental health problems after natural disasters, 2)disaster mental health efforts in recent natural disasters, 3)expected roles of health professionals at local, prefectural and national level in time of a disaster. Finally, as a matter of mental health for all involved, stress management of health professionals was discussed.
Psychological reactions to a disaster are summarized as 1)adaptive reaction, or recovery, 2)normal reaction to the abnormal event, and 3)specific mental disorder. Posttraumatic stress disorder has been a focus of the disaster mental health, however, a broader range of mental health problems including subclinical-level mood and anxiety disorder and suicidal tendency also warrants attention in community health. Thus, the high-risk and population strategies are effective avenues to address public mental health in time of disaster as well as ordinal situation.
Based on the recent effort in disaster mental health services, yet many challenges are raised in clinical care and mental health activities by different professionals. To coordinate the care and service system, the efficient service allocation is required by multi-layered services. Local health professionals play significant roles in safety check-up and secure and comfort in health and hygiene issues, and establish good relationship with the public. In contact with people in mental health crisis, local health professionals are expected to provide psychological first aid and assess person’s needs for mental health professional’s help. To maximize the use of limited mental health resources, service planning and coordination are required at prefectural level, and the service should include allocation of mental health resources(assessment of local availability and needs for dispatched team), high-risk approach, such as identification and care of mentally disabled people in need, population approach such as health education regarding mental health problem, and handling of media attention. At a national level, update of the disaster mental health guidelines, and technical assistance and training are expected to be done by professional institutes. Strategic response at individual and organizational level is required to deal with additional distress among the health service providers.
